Arts Wayland Vendor Agreement

Vendors providing programs for participants under the auspices of Arts Wayland must complete this agreement. If you prefer to complete a paper version we will send it to you by email. Please read below, sign, accept the terms and submit prior to the start of your program. 

Please certify that you have read the Program Guidelines and Requirement document by your signature below. A completed Program Proposal must be submitted before this agreement.

The vendor is responsible for all aspects of administering and running the program, including without limitation registration of participants, collection of fees from participants, and refunding fees if the program is cancelled. The vendor will establish its own policies for cancellations and refunds. The vendor will provide Arts Wayland with a list of participants. The vendor will ensure that each participant signs a Release and Waiver of Liability in the "Check-In" form provided by Arts Wayland on the first day of the program, and will deliver the releases to Arts Wayland. The vendor will comply with any instructions provided by Arts Wayland relating to use of the studio. The vendor agrees to pay Arts Wayland 20% of the total receipts from participants in the program offered by the vendor within 5 business days of completion of the program.

Release and Waiver of Liability

I, the vendor, voluntarily consent to provide programs at the Arts Wayland Studio.

I hereby acknowledge that Arts Wayland assumes no liability for the vendor’s welfare or for any injuries, claims or losses that may arise from participation in this program. On behalf of myself, my heirs, administrators or assigns, I hereby release and forever discharge Arts Wayland, its agents, administrators, volunteers, and members from any and all claims or causes or actions arising from my participation in this program.
